Effect of Perioperative Nicorandil on Myocardial Protection in Patients Undergoing Cardiac Surgery with Cardiopulmonary Bypass, a Retrospective Study

This retrospective study evaluates the myocardial protective effects of perioperative Nicorandil (NICD) in patients undergoing cardiac surgery with cardiopulmonary bypass. Analyzing data from 2406 patients, the study found that NICD significantly reduced the incidence of myocardial injury and postoperative acute liver injury (ALI), and shortened the duration of mechanical ventilation, ICU and hospital length of stay, and chest drainage. These findings suggest that NICD can offer substantial myocardial protection and improve postoperative outcomes in cardiac surgery patients.

Intra-abdominal Hypertension Predicts Worse Hospital Outcomes in Children After Cardiac Surgery: A Pilot Study

This study highlights the incidence and impact of postoperative intra-abdominal hypertension (IAH) in children undergoing open-heart surgery, showing that 24.7% of patients experienced IAH, with significant occurrences within the first 24 hours of ICU admission. Factors like right-sided heart lesions, redo sternotomies, and prolonged cardiopulmonary bypass were identified as predictors of IAH. The condition was associated with increased inotropic support needs, gastrointestinal complications, sepsis, multiple organ dysfunction syndrome, and extended stays in the ICU and hospital. This underscores the importance of monitoring and managing IAH to improve postoperative outcomes in pediatric cardiac surgery patients

Effects of Albumin and Crystalloid Priming Strategies on Red Blood Cell Transfusions in On-pump Cardiac Surgery: A Network Meta-analysis

This network meta-analysis evaluates the impact of albumin and crystalloid priming strategies on red blood cell transfusion requirements in on-pump cardiac surgery. The study reviewed 830 studies and included 10 in the final analysis. The findings suggest that crystalloid priming may significantly decrease total perioperative red blood cell transfusions compared to albumin. However, the difference in postoperative transfusions was not statistically significant. Both direct comparisons and network meta-analysis indicate that albumin priming results in higher transfusion rates, suggesting a reconsideration of its use in cardiac surgery for optimal blood management.

Clinical Use of Bedside Portable Low-field Brain Magnetic Resonance Imaging in Patients on ECMO: The Results from Multicenter SAFE MRI ECMO Study

This study evaluates the safety and efficacy of ultra-low-field portable MRI (ULF-pMRI) for detecting acute brain injuries in ECMO patients across two academic centers. ULF-pMRI was successfully used in 50 patients, showing good quality images of intracranial pathologies. Adverse events occurred in 6% of cases, demonstrating the procedure’s safety. The incidence of acute brain injury was 44%, with ULF-pMRI providing more sensitive detection of ischemic injuries compared to traditional head CT scans. This suggests ULF-pMRI’s potential in improving clinical care and research on ECMO-associated brain injuries.
